We hate to see you go, and would like to offer to place your monthly membership on hold if that would be helpful. We offer all members the opportunity to freeze their monthly membership for up to 3 months at a time, 2 times within a year.

If you would still like to cancel your monthly membership, please complete this form and we will be in touch with you shortly to finalize cancelling your monthly membership. If you do not receive an email confirmation within 3 days, it is your responsibility to followup with Jenn Cedrone at jcedrone@fitnessunlimited.com.
